The forms get staked into the footing trench. Vertical stakes to hold the forms up, kickers and spreaders to keep them spaced and plumb(ish).

I did this last summer. 40' x 16'-6" addition. Footing is 15" x 8". Stemwall is 6" wide by 30some inches tall.
It was my first foundation. I made my forms out of 2x4 and 1/2" ply. Probably... -

We've had a pretty cold spell here, too. My fingers are also the first to go. All "thermal" or otherwise cold-weather-rated gloves suck, IMO. These things go a long way to helping me out.
